Prevalence of hypophosphatemia in the ICU – Results of an international one-day point prevalence survey

Background & aimsHypophosphatemia (HypoP) is associated with organ dysfunction and mortality. Despite its potential severe consequences, HypoP remains poorly characterized in terms of real prevalence timing onset. The primary objective was to determine the defined as blood phosphate <0.8 < 0.65 mmol/l on one particular day at international level.MethodsOne-day point survey conducted by Section Metabolism, Endocrinology Nutrition (MEN) European Society Intensive Care Medicine (ESICM) during week 11–2020.ResultsIn total, 56 adult 4 paediatric ICUs, from 22 countries participated: 41 ICUs were mixed medico surgical, 19 others being cardiac, medical or surgical. Phosphate measurements performed daily 21 1–3 times per 39 ICUs. On D-Day 909 patients (883 adults) present 668/883 (75.7%) had serum/plasma determined, revealing a 103 (15.4%) aged 62 [18 85] years. Of those, 49 presented <0.65 mmol/l: cases hypophosphatemia detected any time patient's ICU stay. No observed children. A treatment protocol existed only 41.1% independently type, size. Only 41/98 (29/41 mmol/l) receiving phosphate.ConclusionHypoP least 15.4% patients, may occur absence repletion protocols 60% participating an unexpected finding, confirms necessity for development guidelines.ClinicalTrials identifierNCT04201899 Hypophosphatemia level.
